Job Description

Regional Hospice and Home Care provides exceptional interdisciplinary care to patients and their families.

Our team is looking for licensed social workers to provide social work services to our New Haven area community based hospice patients as well as those who are inpatient at the Yale-New Haven/Smilow Cancer Center under Regional Hospice and Home Care.

While candidates would be working mostly in the New Haven area, this position will require some orientation, training and on-going meetings at our main location in Danbury.

Evening and weekend availability preferred.

Qualifications must include:

  • Connecticut LCSW certification required
  • Experience in Hospice and/or Palliative Care required
  • Spanish speaking a plus
  • Must have valid drivers' license, proof of automobile insurance, and a clean driving record.

Company Description Since 1983, Regional Hospice and Palliative Care has been dedicated to its mission of providing care, comfort and compassion to those facing advanced illness in Fairfield, New Haven and Litchfield Counties. Our agency provides patients and their families with multi-disciplinary care, pain management, spiritual and emotional support, and volunteer programs for dignity and optimum quality of life during end-of-life care, regardless of their ability to pay.

With the 2015 opening of the Center for Comfort Care and Healing, our patients and their families now also have the option of utilizing these comprehensive, interdisciplinary services in our beautiful, state-of-the-art new Center, a specialty hospital for palliative and end-of-life care. We have the first Center of its kind in the state of Connecticut, and are at the forefront of end-of-life care in the state. While providing patients with the highest quality care 24 hours a day and 7 days a week, we have witnessed families utilizing all of the amenities of our hospice home, spending time with the patient and each other in our welcoming family kitchen, our comfortable living rooms, and our beautiful gardens.

Regional Hospice is the only hospice organization in the state of Connecticut and one of 140 nationwide (out of 5,300 eligible hospices) to be selected by the US Department of Health and Human Services to participate in the Medicare Care Choices Model. This model tests the effectiveness of giving patients the option to concurrently receive curative treatment and hospice care in hopes of improving quality of life at the end of life. The results of this five-year research study will shape the future of hospice care in this country, hopefully enabling more people to access comfort measures sooner.

Most recently, Regional Hospice was selected to be the exclusive hospice care provider at Smilow Cancer Center of Yale-New Haven Hospital, tasked with the responsibility of educating medical oncologists to better understand end-of-life options. Nationally, less than 45% of Medicare beneficiaries use hospice at the end of life, and Connecticut has the lowest rate of hospice utilization in the nation. Regional Hospice continues to work hard to improve this rate through these various initiatives.
