DESCRIPTION: The Psychiatric Advanced Practice Registered Nurse (APRN) is responsible for conducting psychiatric evaluations and medication management sessions for clients for the purpose of prescribing medication as part of the overall client treatment plan. Psychiatric evaluations booked in hour increments and follow ups are a half hour. The Psychiatric APRN works closely with the clinical staff including a Psychiatrist, APRN, Therapists and Case Managers to coordinate care.
REPORTS TO: Executive Director of Clinical Services
SUPERVISES DIRECTLY: None
?? Perform evaluation and management visits and coordinate care for Turning Point clients under the direction of a supervising physician.
?? Assist with care planning and provide follow up during transitions of care.
?? Maintain records of all visits and care coordination activities within the electronic health record.
?? Provide supports for improving overall wellness, managing clients, and identifying gaps in care within the agency.
?? Collaborate with clinical and residential staff to provide best possible care.
?? Other duties as assigned.
?? Knowledge of recovery, mental health and other co-occurring disorders;
?? Experience in working with addiction in a residential and/or intensive outpatient milieu
?? Experience in working with chemical dependency in a residential and outpatient milieu;
?? Experience in crisis intervention and conflict resolution skills;
?? Excellent interpersonal skills and the ability to maintain a positive collaborative relationship among staff;
?? Ability to work in a fast-paced, dynamic environment
?? Active Connecticut License as an Psychiatric Advanced Practice Registered Nurse with the ability to treat a 17 to 30 year old population
