Job Description

Position Summary: Provides intake assessments for all new referrals, support and counseling to agency clients. Completes written and face to face screenings for new potential client referrals. Conducts weekly one to one sessions with all clients. Functions as a liaison and collaborates with all hospital and community incoming referrals. Provides comprehensive planning and active supports for the care and treatment of agency clients. Networks with area community providers and provides case management supports for the benefit of the clients. Performs other duties as assigned.

Job Requirements: Masters Degree Preferred in social work, psychology, or related field required. Prior residential/respite/crisis experience preferred. Bachelors Degree considered if combined with five years human service/case management experience. Valid Connecticut drivers license required with good driving record. Available to work flexible schedule including evenings, weekends, and overnights as required. Full-time.
