Job Description

As a Clinical Case Manager with The Institute of Professional Practice, Inc., you will be in charge of case coordination and clinical case management. You will also be conducting behavioral and therapeutic services, as well as crisis intervention support services. Clinical Case Managers are responsible for the oversight of Professional Parents and the overall home environment. It is important you are able to develop and maintain positive, professional relationships with referral agencies and community representatives.

RESPONSIBILITIES:

  • Completes initial assessments for children assigned to their caseload (behavioral, bio-psycho-social, educational, and other as indicated
  • Provides clinical, consultative, training, and programmatic home based services to the child and Professional Parent
  • Develops and Conducts quarterly and annual Overall Plan of Service (OPS) meetings and reviews for each client
  • Develops behavioral support plans or guidelines for each child on their caseload
  • Actively participates in clinical and administrative supervision
  • Monitors and ensures Professional Parent compliance with all IPP, state and federal regulations regarding licensing approval
  • Participates in on-call rotation, providing emergency coverage to all Professional Parenting Program clients

Requirements:

  • Masters Degree in social work, family therapy, psychology, or special education
  • Licensed or license eligible
  • Minimum of two years of child welfare experience
  • Excellent interpersonal, organizational and communication skills
  • Valid drivers license and safe driving record
  • 18 years or older
  • Computer skills
