Job Description

Mental Health Workers, Recovery Assistants, Community Support Assistants, Recovery Support Specialist & Per Diem Shifts

Position Summary: Provides rehabilitative mental health and substance abuse services to assist individuals to achieve and maintain the highest degree of independent functioning in the community. Works collaboratively to provide intensive rehabilitative community supports, crisis intervention, group and psycho-education, case management activities, person centered goal development. Provides transportation and assists in providing hands-on-instruction with activities of daily living. Performs other duties as assigned. EOE

Job Requirements: CSS II and MHW require a Bachelor's Degree and at least one year prior experience in the human service field. CSS I require Associates Degree in addition to a DARC certification or other identified Behavioral Health Certificate with at least one year prior experience. RA and CSA require a minimum of a High School Diploma plus 3-5 years of work experience in a related field. Additional certifications in CPR and First Aid preferred.

Through consultation with family members, home healthcare providers, clinical and community providers, roommates and friends, the Continuum of Care staff will develop an Individualized Service Plan that identifies specific personal goals and objectives for the individual's recovery.
