PREVIOUS HOMECARE EXPERIENCE IS REQUIRED

The RN Admission Nurse is responsible for admitting patients to service and developing appropriate plans of care. The admission nurse is responsible for accurate and efficient completion of all admission components, including OASIS and comprehensive assessment and establishment of the initial plan of care in keeping with best practice standards.

RN Admission Nurse - ESSENTIAL RESPONSIBILITIES

* Responsible for providing comprehensive assessment of patients, including assessment of response to disease or injury, risk factors, safety issues, function, cognition, sensory status, and ability to communicate and learn.

* Responsible for accurate and timely completion and submission of OASIS data set as part of the comprehensive assessment of the patient.

* Responsible for utilization and integration of best practice standards in the assessment of patients and in development of patient-specific care plans.

* Responsible for the reconciliation of medications, initial medication assessment and teaching and establishment of the medication profile.

* Responsible for initiation of patient teaching and the provision of appropriate agency teaching materials and resources

* Responsible for establishing a plan of care, in conjunction with the patient, addressing all active problems and risks, and establishing goals for care that are accurate and measurable.

* Responsible for identifying the appropriate payer for services in keeping with payer qualifications and conditions, and for providing appropriate notification in situations of non-coverage.

* Responsible for the admission of patients to services in keeping with required productivity standards

* Participates in-service/continuing education as required

* Participates in the orientation of staff members to the agency and its policies and procedures.

* Communicates effectively and tactfully with patients, recognizing their age, cultural diversity, needs, abilities and physical condition.

* Can be depended upon to report to work on-time, use time off appropriately, and complete annual education & medical requirements.

* Performs related and other duties as required.

MINIMUM QUALIFICATIONS:

* Previous Homecare experience to demeonstrate knowledge of State Regulations, Medicare, and Medicaid regulations

* Current Connecticut Registered Nurse License, BSN preferred

* Possess reliable transportation.

* Demonstrates organizational, written, and verbal skills.

* Demonstrates competency with clinical assessment and treatment skills within the scope of practice.

* Demonstrates competency with accurate and timely completion of OASIS data set at all points of care.

* Demonstrates ability to develop comprehensive patient-specific care plans utilizing best practice standards.

* Demonstrates knowledge of payer requirements and conditions for coverage.

* Demonstrates proficiency with computerized documentation and problem solving skills
