Job Summary The Licensed Practical Nurse I is responsible for giving clinical support to providers and providing clinical and supervisory assistance to nursing staff. The LPN I will advocate for patients seeking and receiving care and provide support to Community Health Workers. # CS-HHC#s Expectations of all Employees Adheres to all CS-HHC Policies and Procedures Conducts self in a manner that represents CS-HHC#s Values at all times Maintains a positive and respectful attitude with all work-related contacts Communicates regularly with supervisor about Departmental and CS-HHC concerns Consistently reports to work on time, prepared to perform the duties of the position Meets productivity standards and performs duties as workload necessitates # Essential Duties and Responsibilities Performs screening tests and order routine lab work with medical standing orders Completes specific treatment or nursing procedures as ordered by the provider Assists in providing outreach services to patients who miss their appointments Responsible for placing orders and managing all imaging studies including Ultrasound, CT scan, MRI and Gynecology imaging Performs triage functions of the patients and routes the patient to the appropriate clinical service Administers medications, immunizations and treatments as per written order of the provider. Adheres to the #Medication Reconciliation Act# Responsible for completing telephone triage Provides supportive nursing counseling related to common acute medical problems and to primary preventive care for patients and their care takers Responsible for preparing the patient for the physical exam according to specific guidelines and prepare necessary equipment for the procedure Obtains EKG#s and blood pressure checks to be reviewed by a Provider Provides supportive nursing counseling related to common acute medical problems and to primary preventative care for patients Contributes to in-service education and helps identify areas where needed Attends local medical and educational seminars as necessary Supports the transfer of records to SRS and obtains releases and necessary reports from other care providers and patients Assists with the orientation and training of CHW staff Attends mandatory monthly Nursing supervisory meetings and occasional weekend trainings as required Performs all other duties as required # Qualifications # Education:##### LPN Graduate from an accredited school. Must be registered as an LPN in the State of Connecticut. Current CPR certification from the American Heart Association required. Phlebotomy Certification (National Phlebotomy Association or National Healthcare Association) is required to work at any of the Satellite Sites. # Experience:#### ConnecticutLPN licensure required. Experience in Internal Medicine, Surgical Specialties, Long Term Care or Rehabilitation required. Experience performing EKG#s and triaging patients also required. Experience performing phlebotomy is essential. # Knowledge/Abilities:# Excellent Patient Management skills. Must be strongly motivated and able to accept supervision. Computer skills are essential with the ability to learn AS400, SRS, MD Link and Mednexus. Ability to provide phone triage along with knowledge of referral process, medication refill procedures and Hollister charting are also required. Knowledge of CLIA waived lab testing, Urine Dipsticks and Gucometers are necessary.# Bilingual individual is desirable. # Physical Demands/Working Conditions The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
