JOB DESCRIPTION

TITLE: COMMUNITY COMPANION HOME DIRECTOR

REPORTS TO: EXECUTIVE DIRECTOR

POSITION DESCRIPTION: Perform a wide variety of administrative and management duties under the general direction of the Executive Director, to ensure the smooth operation of the CCH Program. Ensure that the needs of individuals are being provided by the CCH licensees.

DUTIES & RESPONSIBILITIES:

* Ensure that all individuals will receive nursing support services including assessments, preparation of IP's and all associated nursing tasks.

* Assist licensees in meeting all licensing responsibilities, including development and implementation of plans of correction. Attend in-services as requested by the DDS Director.

* Ensure that licensee maintains accurate records as required by regulations.

* Review plan of corrections with the licensee. Develop the plan of corrections, distribute it to the team and send the original plan to DDS.

* Participate as an IP team member for individuals as requested by case managers or nurses.

* Ensure that in-service training to individuals, licensees concerning medical matters occur.

* Ensure that nursing staff function as liaison between medical community and individuals / licensee occur.

* Ensure nursing staff prepare PRC/HRC cases in a timely fashion and attend appropriate PRC/HRC meetings.

* Assist in the recruiting and development of new CCH licensees.

* Ensure nursing staff visits individuals to evaluate injuries and /or illnesses as needed or requested.

* Ensure monthly monitoring by the Quality Assurance Staff is completed as well as the web-based attendance.

* Ensure licensee maintains appropriate records such as incident reports, activities in community, personal funds of individuals, individual absences for more than 24 hours.

* Identify to the DDS CCH Director, issues of non-compliance by the licensees to the regulations.

* Review, and approve automated payroll system for all CCH employees. Approve mileage documentation, and schedules from CCH staff. Approve all PTO requests.

* Evaluate and monitor CCH staff, obtaining timely reports and feedback from CCH staff.

* Facilitate and schedule CCH staff meetings and DDS meetings when necessary.

* Ensure licensees have the emergency on-call number available within the home.

* Interview and make recommendations to Executive Director regarding nursing and any other openings, if any occur.

* Attend DDS meetings as requested.

* Ensure behavioral support is provided.

* Ensure emergencies are resolved.

* Ensure that the attached contractual DDS responsibilities are implemented.

* Perform other related duties as requested by the Executive Director.

* Travel required (approximately five times in a month ? or once a week). Your vehicle maybe required for travel throughout CT.

* Ensure maintenance of company car.

* Audits and submits mileage reimbursements for CCH staff.

27. Submit gas receipts to Accounts Payable.

* Ensures all office equipment (postage meter, copy machines, etc.) is in working order.

QUALIFICATIONS:

* B.A. or B.S. degree in Special Education, human services or 5+ years' experience in a Director role in a related field.

* Proven in-depth knowledge of CT DDS CCH regulations, policies and protocols.

* Experience with development and implementation of programs.

* Excellent computer skills utilizing Microsoft Office products.

* 5-10 year experience in DDS field.

* 5-8 year experience of successful supervisory experience.

* Valid Connecticut driver license and insurable by the agency insurance carrier.
